Transition Strategies - 3 Training Sessions

Transition assessments are essential in the transition planning process, following federal and state requirements. They can be formal (standardized tests) or informal (interviews, observations) and should be chosen based on individual needs. Proper planning and integration into the Individualized Education Program (IEP) ensure effective use of assessment results. A coordinated set of activities, required by federal and state regulations, involves students and families in planning and includes a personal graduation plan. Developing appropriate measurable postsecondary goals, guided by State Performance Plan Indicator (SPPI) 13, is crucial for transition planning. These goals promote self-determination and align with federal and state requirements, facilitating successful transitions to postsecondary education and careers.
